Accepted Insurance
We accept most major health insurance plans including the ones listed below. If you do not see your health plan listed, please contact our office. It is possible that we are in the process of enrolling or may be willing to do so.
We have stayed in contract with insurance companies for patient convenience. This allows us to refer you to specialists, labs, and for imaging. This means we provide notes & coding to your insurance companies so they will cover these other office visits and any necessary medications. Due to these contracts we cannot accept insurance without also collecting the co-payments and deductibles agreed upon when you selected your insurance. This is typical for all membership practices that also accept insurance & this is how our membership works.

All co-payments and deductibles are expected at the time of service. For your convenience, we accept cash, credit and debit cards and checks with proper ID.
